Senior Certified Occupational Therapy Assistant
• As a part of an interdisciplinary team of health care practitioners in a Skilled Nursing Facility, I helped ensure that our patients achieved the highest level of rehabilitative outcomes for an increase in independence with their everyday activities, and an increased quality of life for a safe return home.• Designed facility programs and trained staff members in areas such as neurocognitive impairment, urinary incontinence, and falls and balance, so staff members could employ an evidence-based approach to treatment modalities and patient care. These programs helped with patient safety and decreased hospitalizations by decreasing falls and urinary tract infections.• Worked as a mentor to therapy students engaging in their level I and II clinical rotations, teaching them best practice approaches to patient care, understanding medical compliance, and making sure they were equipped to become entry-level licensed practitioners.• Performed home assessments for our patients and caregivers to assess their environment and how they operate in the environment. By giving the caregivers suggestions for safety and adaptive equipment which could be used to increase the patient's independence with their activities of daily living, our facility oversaw a safe return home of hundreds of patients annually.
