The Senior Director Patient Services, provides continuity of patient care services including Patient Call/Contact Center, Referrals, Tracking, Ordered Services Center, and the Preferred Provider Network. These Centers enhance the Urban Health Plan Quality Assurance efforts and meet the commitment as a Patient Centered Medical Home (PCMH). The Senior Director supports a team-based approach to assure interaction between patients and health care teams in order to achieve the highest delivery of care and is responsible for strategies to enhance the patient experience and to create tactics to measure progress.
