Assure safe, efficient, and cost-effective selection, verification and delivery of medications in a 525-bed community teaching hospital utilizing EPIC• Dose and monitor vancomycin, warfarin, and aminoglycosides daily• Provide drug information and education to medical professionals, students, and patients• Analyze laboratory parameters and monitor drug therapy regimens for contraindications, drug-drug interactions, allergies and appropriateness of drug and dose• Round with the family medicine team serving as the team’s drug information resource• Adjust warfarin regimens based on point of care INR results• Perform order entry and verification of 100+ orders daily in the inpatient pharmacy to ensure appropriate dose, indication, and route of administration. • Aid in the preparation and verification of hospital unit crash carts and code carts to ensure availability of stat medications to appropriate units
