As the Clinical Services Director, I was responsible for the overall clinical oversight of the resident medical care plans which will include training and management of the clinical services staff. Assures plan of care and interventions are evaluated for effectiveness on an ongoing basis and adjusted and updated accordingly. Assures physicians are notified of change in condition; request and orders are processed on a timely basis. Reports / assures timely follow up of signs / symptoms of resident infections. Assists with maintaining thorough and accurate records for each resident. Explains procedures to residents before and during interactions. Responds appropriately to emergencies. Handles linens properly and disposes of linen or supplies in adherence to regulations and guidelines. Follows standard precautions by using personal protective equipment and observes transmission guidelines.

As a clinical care coordinator I was responsible for managing care for the patients of the primary care and internal medicine practice to promote effective education, self management support, and timely health care delivery. Develop and monitor care management processes and support primary clinical teams. Identify high acuity patient population and work to ensure care coordination for this patient population.

As a Staff Registered Nurse, I was responsible for assessing, planning, implementing, evaluating, and documenting the care of my patients by ensuring health history is completed; individualizes patient care plans on admission; identifying discharge planning and education needs; ensuring that intervention or referrals are made promptly; participating with the multidisciplinary team; reviewing patient care guidelines/patient care plan with patient; assists the physician/care provider with procedures and assists with coordination and preparation of patient transfer.

As a PACT RN, I managed care across the health care continuum for the Veterans assigned to the PACT (Patient Aligned Care Team), assuring that care was timely, appropriate, of high quality and cost effective. I worked closely with the primary care provider, mental health providers, and other health care professionals and team members, other clinics, internal and external service, and community agencies. I provided professional nursing assessment, coordination and planning of multiple health care services, acted on behalf of the Veteran to assure that necessary clinical services are received and progress is being made.

As a Telehealth Clinical Technician (TCT), I served as a generalist role, providing technical and administrative support for telehealth programs as an imager and presenter role to support video conferencing, staff training, TMS, telehealth clinical encounters from the patient and provider locations for telehealth store and forward applications, Home Telehealth technology support, for real time telehealth events, including patient education activities, technical support, training, help desk, scheduling support, and other program support duties as needed. As a TCT, I also performed telehealth imaging, including tele-retinal screening, teledermatology, and other telehealth imaging. I was responsible for real-time telehealth applications support for; telemental health, teleprimary care, various medical specialties, and other telehealth programming as it becomes available and other program support duties as assigned. I served as the primary point of contact and technical expert for all telehealth activities within assigned area/location. I was also crossed trained to cover in lab and perform phlebotomy duties which included venipunctures, point of care lab testing, centrifuge blood specimens, and other laboratory duties.

As a Staff Licensed Practical Nurse (LPN) in the float pool, I performed and documented assessments appropriate to licensure and ensured that the assessment is reviewed by a Registered Nurse; ensured a health history is completed; ensured that intervention or referrals are made; identified discharge planning/education needs; provided patient care; reviewed Patient Care Guidelines with patient and their family and provided patient/patient family with appropriate education and counseling; administered medications; performed procedures according to physician order; identified significant changes or high risk situations in patient’s condition. Patients cared for include those in the following age groups: infant, children, adolescent, adult and geriatric in cardiac/telemetry, oncology, medical/surgical, pediatric, womens health and ortho/neuro unit settings

As a Nursing Assistant, I assisted in the admission process by collecting data; collecting information from the patient and/or family and relaying the information to the Nurse; identifying changes in patient’s condition and communicates these changes to the Nurse; performing procedures according to physician order and nursing delegation; assisting physicians or other professionals with procedures; performing personal cares and comforts; transporting and/or transferring patients; demonstrating knowledge of location, set-up, use and cleaning of basic equipment; assisting with the discharge process; completing all documentation; maintaining a clean, orderly physical environment and demonstrating the ability to prioritize workload.
