Comprehensively assesses and anticipates all aspects of the patient’s clinical nursing needs, pain and other symptoms and, in collaboration with primary care physician or hospice physician, develops an individualized plan of care. Incorporates patient/family preferences and goals within the plan of care. Supports and respects the patient’s and family’s unique diversity, including spiritual and cultural beliefs. Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards. Provides patient and family education as appropriate. Areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying. Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members. Engages collaboratively in conflict resolution processes as needed. Flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external. Participates in mandatory educational activities and required meetings. Competent using agency information and technology systems. Help start and co-chaired Nurse Practice Council.

Instruct clinical LPN students in the following areas: clinical situations, oral, intramuscular, subcutaneous, and intravenous medications, side effects, and other nursing implications, treatments and skills such as; dressing changes, PICC and Central line maintenance, Tracheostomy care and suctioning, Wound Vacuum’s, Catheter’s, and NG and feeding tubes, disease process, assessments skills of signs and symptoms of possible disease, proper and effective charting, patient teaching and proper and effective care plans

Assess patients for development of signs and symptoms of possible disease, administer oral, intramuscular, subcutaneous, and intravenous medications, PICC and Central lines maintenance and dressing changes, tracheostomy care and suctioning, wound vacuum maintenance and dressing change, NG and Feeding Tube maintenance, dressing and treatments of wounds, develop care plans for patients with appropriate healthcare team members, educate patient and family as to the progress and treatment of disease, patient advocate, maintain medical records in a compliant and ethical manner and compliance with HIPAA, OSHA, MSDS, Medicare, Medicaid and other government programs
