Neighborhood Health Partners Care Team

-Provides clinical training, clinically supervise staff, manage program deliverables/requirements, evaluate, monitor and direct compliance for applicable regulations.-Facilitates meetings with Accountable Communities Teams to address clinical and social issues, identify barriers and problem solve solutions.-Reviews data reports, analyzes patient utilization and outcome trends for Accountable Communities.

Clinical Integration & Care Coordination

-Collaborate with Greenville EMS Community Parmedic to create patient-centered medical neighborhoods within the Greenville Community.-Care Coordination that reduces unnecessary EMS and emergency room utilization in specific underserved communites.-Provide screening, assessment, planning, problem resolution, resource management, social work services, counseling and crisis management as they relate to the health care and discharge planning needs of the patient/family.

-Assisted department leader with strategy, process optimization, and human capital management.-Collaborated with hospital depts. and medical agencies to develop discharge and post-hospital agendas.-Department liaison for local mental health centers coordinating care for clients that decreases costs associated with length of stays and reduces recidivism.-Assessed and evaluated family dynamics and needs to create treatment plans that incorporate the patient’s/family’s personal, social, and emotional development. -Lead therapeutic interventions with patients/families including individual, marital, parental, family, psycho-educational, and group therapies.
