The Project Street Beat Mobile Health and Prevention Services Manager (MHPS Manager) is responsible for managing the daily operations of the MHC, developing, and maintaining community partnerships, supervising MHC-based staff, and acting as a key liaison between the MHC and PPGNY interdepartmental staff. The MHC Manager acts as team lead for the MHC, which serves NYC communities, prioritizing those with highest rates of HIV and STIs and lacking other accessible sexual and reproductive health care and harm reduction resources. The Manager is an integral part of the MHC Leadership team and works closely with the MHC Clinical Manager and Associate Vice President to oversee all teams and logistics of the MHC operations. The MHC team seeks to engage and center the needs of Black and Latinx communities, women of color, and people at increased risk for exposure to and those living with HIV/STIs, those who are unstably housed and homeless, abuse substances, and engage in transactional/survival sex. This is a full-time position located in New York City at Project Street Beat (offices in Brooklyn and the Bronx), within PPGNY Region One;

Health/Care Coordination - the Program Coordinator is responsible for working with frontline care providers and care coordination staff to promote HIV testing and linkage to care.Work directly with staff and clients in the community or at one, or more, NYP facilities or mobile medical unit (MMU), Participate in daily and weekly multidisciplinary case conferences with program staff from NYP and associated Community Based Organizations (CBOs). Serve as the main point of contact for CBOs for low-threshold linkage of their clients to HIV care or prevention services at NYP or at the program of a client’s choice. Work to link clients to primary care and Health Home services, if indicated. Meet directly with clients to complete an intake into supportive programs, if indicated. Provide outreach and linkage services to selected patients testing positive for HIV, HCV, or in need of prevention services. Provide behavioral and adherence counseling for clients receiving HIV treatment or prevention services.Track and report directly to the NYC DOHMH on new cases, along with cases that are being tracked and traced.Provide administrative support and data management via the NYP dashboard to lead the Rapid Response unit in identifying high-risk patients needing services.

Monitored a suite of medical and mental health providers managing a front desk team to ensure optimal clinical flow.Spearheaded patient intake process & evaluated program eligibility across ADHC, OASAS, Health Home & other services, accelerating workflow efficiency by 20%.Directed timely processing of mental health screenings for new/annual patients, facilitating the generation of 480 qualified leads in EIcare for new patients annually.

Managed an HIV prevention program and developed a 30% growth in program retention via improved curricula development and overhaul of quality practicesOrganized regional conferencing sessions facilitating increased youth engagement by 10%.Coordinated efforts to educate & train 400 LGBTQ+ youths through the development, implementation, & continuous evaluation of outreach & educational curricula.
