CurrentIdentifies in-home service needs, assesses child and family safety conditions, and develops treatment plans according to needs assessments.• Provides counseling, advocacy, case management, and trauma-informed interventions for each client.• Acts as a liaison for clients and long-term service providers as needed.• Mediates and facilitates family support team conferences for each family.• Organizes specialized groups for educational, social skills development, and supportive services.• Prepares program schedules, statistical reports and expedited communication between upper management and staff.
