Responsible for the development and implementation of Individual Service Plans for all assigned residents; assesses residents’ needs and prepares formal assessment summaries for presentation to the interdisciplinary team; schedules and chairs annual, bi-annual and special interdisciplinary team meetings; ensures the attendance of all required team members; promptly writes and disseminates minutes of all interdisciplinary team meetings. Provide direction and coordination to all professional and residential staff in the development and execution of ISPs and training programs; write programs and in-service all appropriate staff on utilization of training programs.Serve as point of contact for correspondence with professionals of therapeutic specialty, which include areas such as Physical Therapy, Occupational Therapy, Speech Language Pathology and Assistive Technology. Oversee the care and implementation of adaptive equipment for all assigned residents. Serve as a liaison with families/guardian, day habilitation & vocational programs and community outreach. Facilitate the coordination of referrals, admissions, transfers, and dismissals of individuals for placement of living arrangements.• Established professional connections with OSU's Wexner Medical Center - Assistive Technology Center to utilize services for assistive technology; focusing on advances for communication and environmental control for those with IID.• Head leader in the creation, planning and presentation of a Disability Awareness Training opportunity to the Chief of Police for the City of Worthington, in efforts to provide premier training programs and resources for crisis intervention in dealing with individuals with intellectual disabilities and autism.• Committee member that has provided valuable feedback in restructuring company policies and procedures.• Professional Development and Certifications: Trained Analyst - Council on Quality and Leadership's (CQL) Personal Outcome Measures (POMs).
