As the Global Patient Navigation Lead I develop, implement and grow strategic solutions that help our clients, across our payer, provider, and public health portfolios, address key challenges inherent in today’s healthcare environment:• Growing Revenue and Expanding Services • Increasing Consumer/Patient Engagement • Improving Medical Outcomes • Improving Profitability Critical to these engagements is moving patients from an unmanaged to a managed state to achieve better care and better health, at lower costs. The solutions I develop and implement accomplish this by ensuring the right care is provided at the right time, in the right setting, targeted at the right medical outcome.Representative Work / Projects:Org Design, Care Team Design/Integration, Change Mgt., and Learning and Development:• Business transformation / cost reduction - mid-size regional provider / integrated delivery networks: led care team redesign, strategic integration and change mgt. efforts, reduced unnecessary ED use by 43%, increased preventative screenings by 13%, and reduced readmissions in critical areas by up to 50% • Client value targeting – regional Medicaid “Blues” plan: developed strategy to engage members and close HEDIS Medicaid gaps in care by 15%. Using analytics, a leveraged care team (clinical & non-clinical), and grass roots / “door-to-door” design to drive results. • Population health mgt. - large national provider: developed diabetes mgt. program focused on patients from underserved communities with unchecked disease. Abnormal A1C levels down by 32%; 40% improvement in A1C test compliance (to 90%); 50% decline in no show appointments (ophthalmology and podiatry exams)• Cost takeout and effective member management - large managed Medicaid payer: developed strategy to manage high risk chronic disease members (top 1%): focus on curbing readmissions, use of the ED and establishment of the PCP/PCMH relationships.
