Rotational clinical pharmacist providing clinical pharmacy services to various different specialties such as surgery, renal and general medicine.One of my most important roles is medicine optimization. This is a holistic approach of reviewing medicine use in patients to maximise the efficacy of drug treatment to treat/prevent disease. Common interventions from medicine optimization involve changing doses, route of administration, withholding drugs, commencing drugs and fluids, and more. All aspects of medicine use are considered and tailored to individual patients. After assessing a patient I liaise with medical staff and other healthcare professionals regarding various pharmaceutical related issues.As a resident pharmacist, I provide an out-of-hours (24/7) medicine information service to support the clinical team and clinically assess appropriateness (and alert prescribers to prescribing errors when needed) of urgent supply of medication. This service covers all clinical areas and involves complex analysis of individual clinical situations to provide a well-researched answer using published resources as well as my own clinical judgement. I have undertaken a number of projects within the NHS (some of which are outlined below). Organisation and time allocation to complete projects by deadlines are managed by myself to fit in with my day to day responsibilities. To achieve this, I create and maintain a meticulous work schedule planned months ahead. With the nature of the job, day to day work load can increase dramatically and requires a highly adaptable approach to alter my schedules.Other duties I perform include the release of bespoke chemotherapy medicines for patients, being the responsible pharmacist for inpatient dispensary, obtaining accurate medication history from patients, researching information for use in Trust guidelines, managing research/project work, mentoring junior colleagues and educating other healthcare professionals.
