Bereavement Coordinator
Current§ Provide emotional support after the death of a loved one to the bereavement designee and continue on-going support for 13 months per Medicare guidelines and company policies. Ongoing support includes but are not limited to phone calls, mailings, grief group referral, and out of area referrals.§ Provide emotional support to staff members as needed or as requested by management.§ Meet compliance requirements including but not limited to condolence cards, monthly mailings, and chart maintenance.§ Work cross-functionally to communication internally regarding plan of care for patient and family including quality assessments and improvement plans for high-risk bereaved.§ Remain on the Psychosocial on-call rotation for weekend coverage.§ Provide training and remain as an available resource to new employees including Chaplain, newly acquired Bereavement Coordinators, and Volunteer Coordinator.§ Implemented and maintain quarterly reporting showing bereavement program performance metrics.§ Established and maintain new grief group effective March 2024.§ Committee chairperson for CAHPS and Scribe for IDG.§ Coordinate and manage bi-annual memorial services.§ Named Employee of the Month: March 2024
