• Advising senior management regarding organizational and IT strategies associated with care management, network development, post-acute care, managed care / risk contracting and new business opportunities• Conceptualizing and operationalizing accountable care models including development of advanced care management staffing models emphasizing team-based care leveraging optimized workflows and IT solutions • Developing value-based strategies and roadmaps to enable population health management strategies leveraging diverse portfolio of clinical programs, resources and digital health solutions and integration points across the care continuum • Facilitating program planning, design and implementation of advanced care management programs for vulnerable populations targeted at transitioning care towards ambulatory environment while simultaneously improving quality / patient satisfaction and aligning with social determinants of health (SDOH)• Leading planning initiatives related to reorganization of acute care and emergency department resources to align with population health management efforts in addition to evolving market requirements related to value-based reimbursement and readmission reduction mandates
