  Physical Therapist 3, UWMC, July 2011 — Present• Primary role in providing service in outpatient Wheelchair Seating Clinic on a daily basis working with patients requiring assistance in obtaining a new wheelchair or modifying a current set-up.• Conduct the process of examining and assessing patient’s needs, trials of equipment and selection of the most appropriate seating components and providing fitting of the wheelchair system with extensive work in custom seating.• Formulate letters of medical necessity (LMNs) in a timely manner to advocate for the patient in obtaining the most appropriate equipment to enable the highest level of independence.• Provide expertise and guidance to colleagues with questions regarding wheelchair seating and device procurement as well as multiple in-services provided to entire staff about different components of the wheelchair evaluation and equipment.• Additionally, work with a very diverse client population including: Spinal cord injury, multiple sclerosis, Parkinson’s Disease, traumatic brain injury, amyotrophic lateral sclerosis, cerebral palsy, Charcot-Marie-Tooth disease, muscular dystrophy, post-polio and CVA on a daily basis performing evaluations, implementing treatment plans and working toward patient centered goals for discharge.• Coverage of multiple settings including oncologic care, orthopedic and post-operative care, ICU-level care and inpatient rehabilitation demonstrating flexibility and ability to cover other units of the hospital as necessary.• Clinical instruction of three students overseeing their growth and improvement, providing guidance and instruction working towards a complete caseload and entry-level independence.
