Support Broker
Current• Assist the Individual or the Individual’s family or representative, as appropriate, in arranging for, directing and managing services that are intended to supplement, but not duplicate, the Support Coordination service.• Serve as the agent of the Individual or family to assist in identifying immediate and long-term needs, developing options to meet those needs and accessing identified supports and services.• Provide practical skills training to enable families and Individuals to independently direct and manage program services.• Provide information on recruiting and hiring self-directed employees, managing workers and providing information on effective communication and problem solving.• Provide support in developing a support and spending plan while providing continuous monitoring of the individual's spending plan.• Develop back-up plans to mitigate potential risks to the health and safety of the Individual.
