Mastodon Solutions is an innovation collaborative started in 2007, focused on development of the Octavia-COMMUNITY software solution to improve patient care through collaboration and coordination of post-acute care delivery across the entire population and community, collaborating without limits. With proprietary risk stratification and automatic identification/alert of care teams via a patented single unified view, our Octavia solution has contributed to the reduction of hospital all-cause readmission by 40% and increased patient experience scores by >11%. We increased detection of Abuse, Neglect & Violence (ANV) and Human Trafficking by 20X (2000%)by automating screening for SDoH and review of suspect cases. Those cases were then permanently tied into special longitudinal monitoring for rapport building and assistance, resulting in tremendous improvements in outcome (incidentally these cases were also found to be the most likely to repeated visit the ED, especially at odd hours, and cause frustration to staff who did not know the underlying reason why- we reversed this). Our root-cause review tools, health literacy initiatives and data analysis suites are driving quality improvement measures with the ultimate mission of readmission reduction, revenue stream optimization and excellence in patient care delivery.
