I have 16 years of experience providing comprehensive case management, psychotherapeutic counseling / intervention, referral, placement and advisory services in managing prevention and support programs for victims of violence, mentally ill, geriatric, frail, disabled and other at-risk populations. I am the liaison between patients and their physicians––facilitating and interpreting communication to coordinate proper care and truly understand each person’s needs. I have extensive knowledge of significant mental illness as well as Alzheimer’s, Parkinson’s, diabetes, COPD, heart disease and other illnesses and the serious implications as they progress, for those who have no one to care for them.With an expert level knowledge of social programs and interventions available, I have an in-depth understanding of MDCD / MDCR and the complexities involved in obtaining authorizations and approval for care and treatment. At this point in my career, I am extremely interested in being a part of an advocacy group or team involved in coordinating multidisciplinary support, evaluating procedures, results, relationships and interactions, and developing policies and proposals that optimize accessibility and improve the quality and timely delivery of care.
