Assessing unmet social needs include cannabis medical card applications, homemaker agencies referrals, living well financially program data collection, and administrative assistance. C4P is an experimental arm of the research study to address patient unmet needs include connecting the patient to local social service organizations, coordinating transportation, navigating complex government and health care systems or integrating the patient more deeply within the C4P clinical team and their communities. The Artful Living Program seeks to connect patients with art, music, education, and wellness opportunities in the community. We aim to provide a wide range of events, such as concerts, exercise classes, arts and cooking classes, and storytelling. Patients are invited to bring their family and friends along so they can share these experiences together.We hope to help patients address other factors in their lives that contribute to their health and wellbeing that might not normally be addressed in the doctor’s office. Our mission is to provide patients with meaningful opportunities that will allow them to develop new skills, connect with others, and improve their own physical health and emotional wellbeing.In this C4P study, the first group of patients can stay with their current doctor or get information about other primary care options. If these patients are hospitalized, they will usually get are in the hospital from a hospital medicine doctor (hospitalist) rather than their primary care physician. The second group will have a doctor who sees them both in the clinic and the hospital. The third group will have the same doctor in the clinic and the hospital and have help accessing community and cultural programs. We will review patients’ medical records and call them every 3 months to learn about their health and the quality of care they receive. Show less
