The Million Hearts campaign was established by promote the use of health information technology to track and improve key clinical cardiac measures and bring together medical providers with communities to decrease the amount of cardiac deaths by one million over 5 years. Coaching practices how to utilizing Electronic Health Records to aggregate reports, interpret quarterly feedback reports which use to benchmarks measures has been essential to building communities of continuous quality improvement. Collaborating with Oregon Health Authority Office’s Health Information Technology and Transformation Center, the Patient Center Primary Care Institute, large Coordination Care Organizations such as Trillium Community Health Plans I have been able to grow Million Hearts campaign efforts and align the measures to existing programs such as the Patient Centered Primary Care Home, Meaningful Use Stage 1 & 2, Physicians Quality Reporting System, Value Modifier incentive programs. I have volunteered to speak at 10 conferences and webinars regarding this project and have consistently provided coherent information and advice to practices on many of these difficult to understand evolving requirements. I have also facilitated an Advisory Council of healthcare leaders across Oregon to learn about new programs, concerns and opportunities for alignment to the Million Hearts campaign. As a result of these efforts there has been a statewide improvement not only on the ability of practices to be able to use their EHR’s to report data but also improvement in all of the clinical measures. Also, as a result of our technical assistance and ongoing webinar series there has been an improvement in workflows and standardized clinical documentation which has been used as a springboard to building improvement in other clinical and operational measures.
