Providing primary care in the homes of people with disabilities or multiple comorbidities that make travel to Healthcare appointments difficult. This service is part of the Nurse Practitioner run clinic supported by The Ohio State University College of Nursing.

In this position, I provide primary care & medical home services to people with some type of disability which makes visits to a primary care provider difficult. Further, I monitor & facilitate growth of the program, provide guidance & oversight of quality & efficiency. This program creates access to healthcare for medically complex, frail and underserved people.
