Case Manager
- Conduct intake assessments, develop and oversee treatment plans for individuals involved in the Assisted Outpatient Treatment (AOT) program. Establish and maintain an ongoing therapeutic relationship and caseload.- Provide ongoing monitoring of individuals in the AOT program to ensure compliance with court-ordered treatment planning and medical court appearance. Supervise, organize and support recreational and/or socialization activities.- Provide linkage and service coordination to individuals to appropriate/court-order treatment modalities. Assist with the integration of mental and physical healthcare and developing a Psychiatric Advance Directive.- Communicate observations and information regarding the client to the AOT Director, Psychiatrist, and treatment team. Meet all expected productivity levels, in regard to direct service provisions to consumers. Document observation, information, and services in progress notes. Monitors service delivery to meet program participants changing needs.- Assist clients with obtaining public entitlement and benefits such as Social Security, Medicaid, Medicare, Welfare, Food-stamps and Section 8. Provide transportation, support counseling, and crisis intervention services as needed.
Direct Care Professional/Family Counselor
- Facilitate cognitive behavioral therapy sessions with at-risk children that experience serious emotional disturbances. Develop comprehensive assessments and treatment plans 30 days after admission and review plan every 90 days.- Monitored medication regimen for clients and schedule medication monitoring appointments with program psychiatrist. Maintain required clinical and administrative documentation, and submit weekly observations on clients.- Immediately advise appropriate personnel of evidence of child abuse or neglect, and/or conditions or situations which compromise, or potentially compromise, service to clients. Assist children directly to address self-care needs.- Establish a therapeutic milieu, provide group counseling, individualized interventions, and support as directed by the treatment plan. Provide activities to address daily life. Participate in the development of discharge plans and make needed referrals.- Plan and implement therapeutic experiences to address and meet each client's unique needs, strengths, readiness, and skill levels. Employ a variety of accepted techniques and strategies to achieve goals mutually established by the client and the treatment team. Assess the psychosocial functioning of children and families referred to the program.- Received Nonviolent Crisis Intervention (NCI) certificate, earned through crisis prevention training.
