To reflect upon my own nursing practice in recent years, I would consider one of my greatest strengths is as nurse teacher. I have been able to develop my own teaching skills and apply this through my professional nursing practice both within individual assessment and informal teaching sessions. My learning development and sharing of expertise with patients supports their own learning and thus their own informed choices regarding behaviour change and identification of the importance of activities of daily living; those which matter most to them. Reflective learning is fundamental to effective nursing care provision and in considering my own approach to self- learning and the development of my own nursing practice, I continually adhere to this method of learning in order to enrich a specialist nursing approach which is based upon high standard individualised holistic care; supporting the whole person with self-management of heart failure and provide the coping mechanisms required in order to successfully achieve this.My interest within cardiac nursing commenced during pre-registration nursing placement; within Acute Coronary Care and the transplant unit at Harefield Hospital, thereby influencing the choice of my extended literature review exploring the psychological influence of patient illness beliefs and the associated recovery process subsequent to treatment intervention; 'Does the development of 'hi-tech' cardiac intervention have a detrimental effect upon a patient's illness beliefs and subsequent adherence to cardiac rehabilitation?'

My interest within heart failure nursing had developed during my role as a cardiac rehabilitation specialist nurse. Through mostly self-guided learning, I have been able to effectively teach and support patients who are learning to manage their condition within a heart failure workshop; patients are given the opportunity to take some time within a group setting to discuss the self-management of their condition, further exploring both the physiological and psychological impact of heart failure. Through patient feedback and reflection, I have tailored my practice to focus nursing care delivery towards the individual person rather than facilitating nursing education which focuses uniquely upon heart failure classification and management.Heart failure management is much more of a continued individual nurse and patient partnership and in fulfilling the main duties and responsibilities of a heart failure specialist nurse, I consider my experience and development over the last ten years, both professionally and personally, to have provided me with the skills to perform this nursing role successfully in order to continually deliver a high standard of specialised nursing care provision.I consider my nursing development and experience during the differing stages of a patient’s cardiac journey have provided the opportunity to transfer my nursing expertise to build upon specialised heart failure nursing which importantly should be based upon effective communication and respect in order to deliver individualised professional nursing care provision. Having had the opportunity to develop my nursing career both within acute setting and then to support a patient with long term management of their cardiac health, my heart failure nursing role is an opportunity to demonstrate and value my own good interpersonal skills and organisational ability, working well both as a team member and leading independently as an autonomous practitioner

my duties as a cardiac rehabilitation specialist nurse are to provide specialist nursing skills and knowledge as part of a multidisciplinary team in order to deliver a community cardiac rehabilitation for patients following a recent cardiac event, cardiac procedure or to support patients with heart failure. The nurse role within the team is to provide a comprehensive and holistic assessment of patients, using investigative and analytical skills in order to plan and deliver a treatment plan, which is supportive of self-management and achievement of set goals identified by the patient.Nursing intervention encompasses accepting clinical responsibility for patients and maintaining a high standard of clinical care, continually assessing clinical risk management throughout the programme.Being an educator is fundamental to the nursing role, teaching and communicating effectively with patients to facilitate progress in cardiac rehabilitation and to ensure an understanding of their condition so that patients are supported in the short and long term with self management of their condition.
