Provided support and programme management of the Primary and Community Care Model of Care for COVID-19. This included collaborative oversight of the development and implementation of national standards and model of care. Worked with a team responsible for providing evidence to inform service delivery and decision-making across the range of public health function including facilitating strengthened relationships, engagement, and communications nationally, regionally & locally in relation to clinical networks whilst using data and insights to identify trends, risks, and opportunities, to influence and guide organisational and system-level decision making process.My role entailed a mix of policy advisory, operational excellence, and stakeholder management working closely with clinical specialists and advisors managing community-based systems of care, agreeing health and wellbeing plans, executing those plans, and monitoring outcomes which then feed into the design
