Part of my Post Graduate Training. Included both Clinical and Public Health Roles.Clinical Work involved postings in the Department of Anaesthesia, Emergency Medicine and Psychiatry. The programme involved postings at Community Health and Development ( Rural), Low-Cost Effective Care Unit (Urban), the Rural Unit for Health and Social Affairs ( Rural) and the CK Job Tribal Health Centre ( Tribal).The base hospital was a 120 bedded hospital run by the Department of Community Health and Development. The hospital serves as a secondary hospital that has two obstetricians, two pediatricians, one anesthetist, one Internal Medicine Physician, and two family physicians. The hospital has one emergency room, one labor room, in-patient wards, and an outpatient department. The outpatient department sees around 200 Outpatient a day. There are special antenatal OPD's that see around 400 low risk and 100 antenatals a week. The labor room conducts an average of 3500 deliveries a month including caesarian sections. Special high-risk pediatric OPD is held daily and is manned by a post-graduate registrar as well as an attending pediatrician. Immunization OPD's are held every Wednesday. A 15 bedded nursery / NICU is also present. The hospital was awarded as a "Public Health Champion" in 2015. The hospital has an excellent primary health care system covering 88 rural villages and 150 tribal hamlets. Every village is visited once every 4 weeks by a mobile clinic manned by a postgraduate registrar as well as an Intern. The mobile clinic allows patients from the villages to avail health services directly from the mobile clinic. Low-risk antenatal can receive all their checkups at the village itself. High-risk antenatals are referred to the base hospitals. Public Health training involved work in Epidemiology, Qualitative Study Designs, and Health Economics.

This position involved full-time clinical work where we trained under Internal Medicine, Obstetrics, Paediatrics and Family Medicine in a small 120 bedded hospital. This involved managing the emergency department, the labour room, the wards and the out- patient department. A lot of focus was also on mother and child health with plenty of exposure to obstetrics and neonatology.

This experience was eye-opening in understanding the ground realities of rural India. It was a hospital in rural Tamil Nadu. Apart from sharpening clinical skill the experience also helped in understanding the importance of culture and tradition in Rural India and its relevance to health. I worked with the Department of Orthopedics as a Junior Doctor assisting a team of three Orthopedicians in the emergency room, the outpatient department as well as in the Operating Room.
