• Use Salesforce software to schedule or check-in 20 or more patients a day for their 1st or 2nd dose of Moderna Vaccine.• Coordinate with Nurse Practitioners and Registered Nurses evaluating and monitoring patient after injections.• Greet and screen each patient entering facility, making sure they feel welcome and are well informed of any weakened immune responses or comprising conditions• Work with colleagues to accommodate individuals with disability, limited mobility and language barrier to properly get vaccinate• Connect elderly and disable clients with cab transportation to get home safe and seamlessly

• Aid various elderly and disable clients with dementia and Parkinson disease ranging from age 60 to 94 with daily living throughout NYC.• Develop care plans and schedules in collaboration with Registered Nurses and clients' families to provide patient centered service• Accompanied patients on weekly outings, i.e., shopping for groceries, clothing and escorting them to routine doctor visits• Prepare nutritious meals (breakfast, lunch and dinner) according to client preferences and dietary restrictions.• Monitor client’s intake of medication and assist each client in maintaining personal hygiene by bathing and grooming them.
