As it was a short-term contract I had to very quickly learn about the HITH program and the numerous issues related to why the patient occupancy had reduced over the last few years. During the term of this contract I worked closely with the Nurse Unit Manager HITH and the Associate Program Director Ambulatory and Community Care to achieve the following which has provided a platform for the HITH staff to leverage off of in the next phase of the project.• -
Manager General Practice EngagementEastern Melbourne Phn Aug 2015 - Apr 2016East Burwood, MelbourneI was responsible for transitioning the practice engagement staff from three medicare locals into one organisation - the Eastern Melbourne PHN. This required me to lead a team of 15 staff to engage with General Practitioners and general practice staff across the region providing support to each team member who were required to function in a different structure from the previous medicare locals. The team was responsible for reviewing and developing updated information which reflected the new PHN organisation and the services that my team would provide. Our target goal was to ensure that all practices within the large geographical region had been contacted and informed of the new organisation which replaced the medicare locals. This we achieved within the timeframe. -
Manager Ml Integration & Hospital EngagementInner East Melbourne Medicare Local Nov 2013 - Aug 2015East Burwood, Melbourne VictoriaI hold several roles at the Medicare Local, including HealthPathways Melbourne Manager, Practice Engagement Manager and Hospital Engagement Manager. This role enables me to combine development of HealthPathways with significant direction and input from the following Local Health Networks: Eastern Health, Royal Melbourne Hospital and St Vincent's Hospital. At the same time the Practice Engagement Team are in an excellent position to promote HealthPathways to the General Practitioners and their practices when on engagement visits. Other work I am engaged in with Eastern Health and a lesser extent the Austin Hospital is introducing and supporting General Practice in Advanced Care Planning services for their patients. The Chinese community is also an area that my Practice Engagement team have been working with during 2014-15 - this work is related to Hepatology and Advanced Care Planning. The Practice Engagement team has been actively involved with supporting Hepatitis B & C initiatives in the region and is currently supporting the CATCH program offered at Eastern Health. This program offers a mobile fibroscan service to General Practitioners, allowing them to identify patients who are at risk or have developed hepatocellular carcinoma. My team also provides a Quality Support Improvement Package to General Practices and will help each practice to navigate through the PDSA cycle to achieve positive patient focused outcomes and gain RACGP points. Additionally, the team led by me, assist a number of key external stakeholders to conduct various projects within the primary care arena which includes but is not limited to General Practice. During the past 20 months my team have built up strong, collaborative relationships with all Local Health Networks in the Medicare Local region including Eastern Health, Austin Health, St Vincents Hospital, Royal Melbourne Health, Northern Health and the private health services including Cabrini, Epworth, Knox Private. -

Research And Policy AdvisorAustralian Nursing & Midwifery Council Jan 2006 - Jan 2008Canberra, ActI managed two main projects for the ANMC, firstly the National ‘Codes Project’, this was a review of the existing Code of Ethics for Nurses and Code of Professional Conduct for Nurses and development of the Code of Ethics for Midwives and Code of Professional Conduct for Midwives. This project completed phase one in February of 2007 and moved into the second phase in May 2007 until the completion and national publication in August 2008. The second project I was responsible for was the ANMC National Guidelines on Professional Boundaries for Nurses and Midwives, which commenced in February of 2007. Additionally, I worked with the Industry Skills Council on their project to produce a national curriculum for Enrolled Nurses. This package offered Certificate IV, Diploma and Advanced Diploma. I was a member of the Industry Skills Council’s working group to examine and progress a renal unit for the Enrolled Nurse and Aboriginal Health Care Workers Advanced Diploma, which was included in the suite of curriculum documents. In my role as Research and Policy Advisor I was also responsible for reviewing the ANMC position statements, working closely with all the peak nursing organisations and the nursing and midwifery regulatory authorities throughout Australia as well as the individual nurses and midwives. I was regularly invited to represent the ANMC on working groups and steering committee to give advice from a regulatory perspective.The Codes and Professional Boundaries are part of the regulatory framework which supports and guides nurses’ and midwives in provision of healthcare. They outline the nursing profession’s commitment to respect, promote, protect and uphold the fundamental rights of people who are both the recipients and providers of nursing and health care.
