The Campbell Center for Collaborative Care is dedicated to the promotion and development of population health and care coordination. In 2010 the center was established in memory of Jack and Joyce Campbell after their untimely deaths ten months apart due in part to medical misdiagnosis and the lack of coordinated care. The primary goal of care coordination is to ensure that patients’ needs and preferences are achieved through physician-patient engagement. Hi, I’m Denise Campbell and I lost my parents to a serious misdiagnosed illness in 2009 just 10 weeks apart. Since their untimely deaths, I’ve been more determined than ever to help people prevent serious illness and disease through a better understanding of health equity, health literacy, and coordinated community care. My parents owned a small business and were passionate about America’s health care crisis and became staunch promoters of prevention and wellness. As compassionate philanthropists, they organized and founded a non-profit foundation, raised funds for numerous organizations, and sponsored several underprivileged children. They gave unconditionally of themselves, their resources, knowledge and time. In following in their footsteps, I want to continue the legacy of wellness that my parents could not finish in their lifetime. In their passing my parents left, in my care, a generous investment fund. I couldn’t find a better way to honor them, than by establishing the Campbell Center for Collaborative Care.
