Facilitates community integration and inclusion by locating activities of interest to individuals supported while exploring alternate avenues of interest. Provide training, supervision, assistance and monitoring in areas related to the acquisition and retention of daily living skills and self-care skills. Responsible for the development and monitoring of the Residential Habilitation Plan (RHP) and the Individual Plan of Protective Oversight (IPOP). These plans should be developed in collaboration with the team to assist the individuals with fostering independence. Monitor the data collection and ensure the daily billing is submitted in its entirety weekly.
