I am a Community-Based Pharmacy Residency trained pharmacist with experience ranging from independent pharmacy to health-system based ambulatory care.I am passionate about using my medication expertise to address social determinants of health that impact underserved communities through education and patient care.Key highlights of my journey towards becoming a greater community-engaged pharmacist, patient advocate, and educator:- In depth ambulatory care practice in family medicine, internal medicine, and geriatrics clinics under a variety of practice settings, including collaborative practice. Served refugee, immigrant, and indigent populations to ensure they receive the care they deserve during transitions of care and in primary care services.- Coordinated the success of multiple COVID-19 vaccine community clinics focused on equitably distributing the vaccine to underserved areas and to communities disproportionately impacted by the pandemic through coordination with pharmacy leadership, health department, and multi-disciplinary team of vaccinators.
