• Responsible for the logging, invoicing and processing of medical records and release of information requests for health services provided by the company.
• Receive and log all medical record requests (verbal and written) from patients, attorneys, Social Security Administration and other health care facilities.
• Generate invoices for records requests where applicable
• Accurately complete all documentation related to record requests such as affidavits
• Assimilate all documents indicated in the request such as the patient’s health record, imaging records and billing records
• Process release of information forms and ensure that the document is scanned into the patient’s account

Scanned and indexed medical records into the electronic medical chart for 12 different Physicians. Released medical records to patients, law offices, other and medical facilities. Retrieved medical information from patient portal and input into the computer/patient medical chart. Made appointments, verified benefits and answered phones/distributed messages.
