As a Care Coordinator for Enhanced Care Management (ECM) at BACS, I work alongside the most vulnerable Medi-cal members with prominent medical and social needs by providing person-centered care coordination and case management. My key responsibilities include assessing the needs of my clients, facilitating personalized and sustainable health action plans, advocating for their care, working together to learn sustainable and independent skills, researching and referring clients to community resources, building their care team, and responding to crisis situations.

I work directly with kids who have experienced extreme trauma and are on the Autism spectrum. As part of their care team, I support the youth to help them feel safe by building skills and advocating for their care. They have taught me values of resilience and being open. Here, I have worked with diverse groups of people, from children in the foster system to kids with large extended families. Due to the nature of the “Crisis Stabilization Home”, in order to be effective I have learned to build trust and rapport with the kids and guardians in a timely and effective manner. I embody a positive, validating and curious perspective.

I work with a team dedicated to understanding and addressing prevailing issues at Cal Poly through Bystander Intervention, such as hazing, gender based violence, alcohol abuse and more. I now understand the need to create an empathetic college community within our current competitive environment. I have learned about the importance of Bystander Intervention in creating a culture of compassion and ensuring students leave Cal Poly as productive members of society, not just academic scholars. I am the Project Manager for the EmpACT module- a workshop designed to inform different populations on campus. Currently this program is about to be reviewed by the IRB and I hope its future implementation will transform the Cal Poly community.
