• Working with individuals in the completion of intake assessments, maintain expected occupancy rates, and ensure staff coverage in designated programs.• Oversee several programs to ensure daily operations achieve program goals and objectives. • Developing and overseeing the implementation of Individual Habilitation Plans, treatment plans, Adaptive Behavior Scales, and other assessments and will provide case management and direct care services for assigned clients, as needed for the program. • Collecting individual’s data from support coordinators for annual individual support plan (ISP) and uploading into PROFILER database and making sure billing for correct services. • Following up with clinical and nursing team to make sure accurate assessments are completed and collect the results.• Training staff on accurate methods of documentation.• Ability to work with members who exhibit behavioral problems while maintaining a calm and positive demeanor.• Assist with planning and coordination of programs and their activities• Monitor implementation of program policies and practices• Work to keep programs on schedule, within stated budgets and functioning smoothly• Support program growth and development as necessary

- Maintain records into Homeless Management Information System (HMIS) regarding case management/counseling activities and termination as appropriate; prepares required reports - caseload reports, case logs, etc.- Interviewing case managers for upcoming positions.- Evaluate, monitor and advocate an array of services to meet client's needs and functionwith DCP&P.- Maintain a constant and consistent relationship with clients in accordance with programrequirements to ensure they are reaching self-sufficiency.- Provide continuing ongoing assessments of client needs, develop treatment plans,coordinate services, monitor client progress, and perform periodic reevaluation of plans.- Development and implementation of service plan that incorporates individual needs andassessments.- Assist homeless families in securing and moving into permanent housing.
