As part of the Integrated Care Team, assigned to a Senior Care Option (SCO) plan, providing assistance to low-income elders and disabled adults, helping them to live independently in the community. I conduct initial and ongoing assessments on new SCO-Senior Whole Health members/enrollees. Managing a caseload of 100-120 clients, which entails determining clients’ eligibility by assessing their needs with a combination of home visits, telephone contact as well as monitoring client’s ongoing services. Completing the required assessment tools and computer data entry in SCO member’s database and SAMS, which includes but is not limited to demographic data, programs/services and Progress Notes. Collaborating with SCO's RN to develop community care plan, arranging, coordinating, and authorizing the provision of appropriate non-medical services. I am a liaison between the SCO and ASAP Care Management.
