Utilizing CHCANYS Statewide data warehouse, analytics and reporting system, practice facilitators drive implementation of HIT-enabled population health management, team-based care and proactive patient engagement. Implementation resulted in a more than 60% increase in FQHC use of nascent population-level analysis to demonstrate the value of the FQHC model.● Restructured the CQI program, including controlled growth through strategic expansion of funding, the development and implementation of a modular staffing plan to facilitate efficiency, teamwork and workload distribution while creating ladders for promotion and professional development opportunities; and establishment of an organizational reporting and grant management structure to facilitate effective oversight.
