In this role I work with a diverse group of members to provide MTM services and to connect them with additional resources to help with specific health concerns. I provide comprehensive medication reviews to members in the community, members in long-term care facilities, and members who do not speak English. I review medication regimens and compare them to current guidelines to determine the appropriateness of therapy and if additional therapy is warranted. I also offer cost saving options to members when available. In this remote role I am often responsible for managing my schedule and doing outreach to schedule visits. I specifically focus on members in long term care during down time since I have over 5 years experience working with this population. My persistence and determination have allowed me to reach many of these members and their nursing teams to complete medication reviews.I also support the adherence measure by spending one day per week outreaching to members who have been identified as behind schedule on a target medication. I look for solutions such as automatic refill, mail order pharmacy, or having the doctor write an updated prescriptions. If members have coverage for additional services to get help with medication set-up I will refer them to their case manager for additional assistance.

In this role I review a variety of patient information icluding medication list, immunization history, recent lab values, and diagnoses. While reviewing this information I check to see if the patient is up to date on vaccinations, if all medications have a clear indications, and if the medications are appropriate for the patient's conditions.After the initial review I call the patient and discuss all the information I have available. I ask about potential side effects or if cost is an issue. I also discuss the patient's current lifestyle and make recommendations where appropriate. I ensure the patient knows what their medications are used for. If needed I send recommendations for therapy changes to the patient's provider. After the call I complete thorough documentation about what was discussed and recommended to the patient so the encounter an be submitted for billing.

In this role I reviewed medication orders for new and established patients at a variety of skilled nursing facilities, assisted living facilities, and group homes. I worked directly with nursing staff at these facilities answering questions and addressing a variety of medication issues. I also served as a reference and a sounding board for providers at our skilled nursing facilities.I prioritized tasks on a daily basis to ensure our delivery runs left on time. I took the initiative to be cross-trained in all roles within the pharmacy. When staffing shortages occurred I cross-trained other team members as well.

At this lower volume pharmacy I had the time to develop personal relationships with my regular patients. This relationship created trust between myself and my patients and allowed me to have candid conversations about their medications. I counseled patients and explained insurance ( deductibles, prior authorizations, etc.) in an easy to understand manner. I also administered a variety of immunizations to help keep patients healthy and meet company goals.

At this high volume pharmacy I was responsible for reviewing new orders and refills, counseling patients, giving immunizations, answering questions, making OTC recommendations and answering phone calls. I quickly learned how to effectively manage my time so that the most urgent tasks were completed in the shortest time frame. I was often the only pharmacist on duty and as such I had to lead my technician team to work as efficiently as possible. During my time in this role we had several new technicians and i helped train them in the various pharmacy tasks they would perform.
