Currento Provided comprehensive, continuous, and personalized care to diabetes, hypothyroid, and hyperthyroid patients in the Division of Endocrinology, Diabetes and Metabolismo Developed a personal niche to provide care for patients who are often deemed "challenging, non-compliant patients" by our standard of care and for patients with intellectual or physical disabilityo Spearheaded a healthy lifestyle program for our diabetes patients by combining Centering Diabetes Medical Group Visits in collaboration with UC Primary Care and UC Integrative Health with a virtual/in person Teaching Kitchen program in collaboration with La Soupe organization, a Board of Health Certified kitchen that utilizes a chef-based model and volunteer network to rescue perishables from farms, grocers, and wholesalers, fighting food waste and food insecurity. Each Centering Diabetes Group Medical Visits involves a curriculum that reflects current evidence and diabetes management practice guidelines focusing on diabetes education, nutrition, physical activity, and stress management. This program strongly complements our diabetes education and medical nutritional sessions to help patients better manage their diabetes, transforming healthcare to impact the lives of patients and their families with an upstream medicine approach. o Received a community outreach funding donation from Kroger/84.51° as the mission of the Teaching Kitchen program aligns closely to Kroger/84.51° values
