My role involves supporting adults in the community with complex needs resulting from a range of different physical and learning disabilities. This is achieved by offering a holistic assessment of a client’s needs including their support network and family in a person-centred way. I use a strengths-based and solution focussed approach to help achieve the desired outcome for the clients I work with collaboratively. I advocate for service users rights and engage in multiagency working. Working within the locality team and hospital discharge has allowed me to work closely with GP’s, CPN, occupational therapists, physiotherapists, speech, and language therapists. As part of this role, I am involved in the process of continuing health care to establish funding pathways for care and support on discharge from the hospital.As a social worker in this setting, I use The Self Directed Support, Adult support and protection and Adult with incapacity daily to guide my practice and have an in-depth knowledge of the legislation that underpins the profession.As part of this role, I am involved in carrying out safeguarding investigations, managing a complex caseload and participating in integrated working supporting clients to reach reablement goals. I use the ‘home first’ philosophy helping service users to remain as independent as possible.I am continually aiming to progress my professional development and practice skills to help me achieve the best outcomes for the clients that I work with. I have a great knowledge of social diversity and equal opportunity issues and can demonstrate this throughout all aspects of practice, with a great understanding of promoting privacy, confidentiality and dignity whilst empowering individuals and enhancing their independence.
