Pioneer Physicians Network is a network of independent physician practices located in the Greater Akron/Canton area. The VP of Professional Standards and Governance is responsible for development and implementation of Board approved policies, plans, and procedures, and to help maintain high professional standards among all providers and staff at Pioneer Physican Network. Other duties include monitoring and evaluation of all Quality Assurance, Utilization Review, and Risk Management activities, while collaborating with the medical director regarding quality of medical care issues. The VP of Governance coordinates meeting with new groups to assist with onboarding, meeting with individual physicians to address non-compliance and disruptive behaviors, and develop corrective action plans as needed. Review and discussion of site and administration personnel issues with HR is also required.

The FPM Medical Editor determines the editorial content of the Journal and makes final acceptance decisions for all manuscripts that enter the Journal’s peer review process. The Medical Editor also manages the Editorial Advisory Board and directs its activities, including the vetting of a content plan that the Medical Editor develops in collaboration with the editorial staff. FPM, formerly Family Practice Management, is a peer-reviewed journal published by the American Academy of Family Physicians. Through its bimonthly print publication, weekly e-newsletter, and extensive website, FPM gives family physicians practical information to help them maintain efficient and effective practices, enhance the patient experience, and maximize their professional satisfaction.

I am on the volunteer faculty of NEOMED, where I'm active in the teaching of Medical and Pharmacy students.

I am a volunteer member of the the NQF's Admission and Readmission Standing Committee, as a representative of the American Academy of Family Physicians. The Standing Committee’s primary work is to evaluate the submitted measures against NQF’s standard measure evaluation criteria and make recommendations for endorsement. The Committee also: • oversee the portfolio of measures• identifies and evaluate competing and related measures• identifies opportunities for harmonization of similar measures• recommend measure concepts for development to address gaps in the portfolio• provides advice or technical expertise about the subject to other committees (i.e. cross cutting committees or the Measures Application Partnership)• ensures input is obtained from relevant stakeholders• reviews draft documents• recommends specific measures and research priorities to NQF Members for consideration under the Consensus Development Process (CDP).

PHILOSOPHY The Primary Care Transformation Committee (PCTC) provides direction and guidance to NewHealth Collaborative regarding advanced primary care transformation and patient-centered medical home development (including quality improvement, coordination of patient care processes and expectations, and implementation of office workflow, processes, and policies). The PCTC also supports achievement and maintenance of recognition by the National Committee of Quality Assurance (NCQA) patient-centered medical home recognition program. PURPOSE The purpose of the PCTC is to: • Receives information on NewHealth Collaborative (NHC) goals and strategies from the Clinical Performance Committee (CPC).• Provides operational and tactical direction to NHC practices to achieve NHC goals.• Provides feedback to the CPC on transformation gaps and opportunities as well as recommendations for plans of action to improve population-based care.• Facilitates education and mentorship opportunities around transformation activities.• Recommends and oversees the development of workgroups, and other opportunities to foster the collaboration of the NHC physicians, practitioners and other healthcare professionals to continually advance primary care transformation.• Ensures the adoption of primary care transformation with a focus on operational implementation and clinical performance improvement.

The Patient Centered Medical Home Committee (PCMHC) provides strategic direction and guidance to Summa Health System's NewHealth Collaborative regarding patient centered medical home development, including quality improvement, cost reduction and coordination of patient care processes and expectations, based on the National Committee of Quality Assurance (NCQA) patient centered medical home recognition program, standards and guidelines.

NewHealth Collaborative (NHC) is a clinician-led Accountable Care Organization (ACO) that partners with communities to compassionately care for and serve our populations in an accountable, value and evidence-based manner. The medical director is responsible for leading the clinical team, and development of care protocols, quality improvement programs, and metric measurements.The Patient-Centered Medical Home (PCMH) is a redesign of how primary care is organized and delivered, transforming it into a team based, highly coordinated approach. Summa Health System's accountable care organization is forward-thinking and recognizes the PCMH a a foundation upon which high quality care is built. The PCMH Lead Physician is charged with overseeing the recognition and transformation of all primary care practices into NCQA Level III patient-centered medical homes in the ACO. This includes leading a team of process engineers and business analysts, helping to develop quality metrics, redesign of office workflows and procedures, and publicly presenting the progress.OUTCOMES• Successfully guided over 50 practices on their way to NCQA PCMH Recognition• Developed a robust platform of primary care practices, leading to improved quality and savings• Developed a culture of quality among providers in NHC• Successfully developed and implemented primary care transformation strategies, focusing on enhanced access to care, team-based care, pre-visit preparation, and population health at the practice level

NewHealth Collaborative (NHC) is a clinician-led Accountable Care Organization (ACO) that partners with communities to compassionately care for and serve our populations in an accountable, value and evidence-based manner. The Board is comprised of a majority of physicians and I was one of two primary care physician directors. NewHealth Collaborative is the Population Health arm of Summa Health.

Summa Health Network (SHN) is the physician hospital organization (PHO) affiliated with Summa Health System Hospitals (SHSH). SHN contracts with insurance companies, health maintenance organizations, preferred provider organizations, employers, third party administrators and other payors on behalf of SHSH and their affiliated physicians.SHN offers a comprehensive network of providers in multiple counties in Northeast Ohio, including Summit, Stark, Medina and Portage.SHN is also a member of the Cleveland Health Network (CHN). CHN is a network of regional PHOs that contracts with managed care organizations. As a member of SHN, physicians also participate in the contracts through CHN.

The System Board Quality Committee (SBQC) is responsible for working with System Leadership and the System Board to set overall System-wide strategy and priorities with goals to advance the clinical enterprise and improve patient safety and outcomes (clinical and experiential) effectively and efficiently. It oversees and drives integrated efforts within and between System entities around these priorities. The Committee's scope of responsibility is all of Summa Health System businesses or entities, including hospitals (owned and affiliated), SummaCare, NewHealth Collaborative, owned physician practices, and the PHO.

The Care Delivery Board, which was absorbed into the Summa Health Board of Directors in late 2016, was the fiduciary board responsible for the oversight of care and service provided by the all of the Summa Health System's hospitals. It was responsible for oversight of all care delivery functions and their regulatory oversight (Summa hospitals, ambulatory sites, joint ventures and affiliate partnerships).Duties: To oversee legal/regulatory fiduciary duties as enumerated by the Ohio Department of Health, Joint Commission and the Conditions of Participation for Medicare and Medicaid. Meetings are anticipated to be held monthly.• Oversee quality and patient safety• Adopt, approve and monitor vision statement of hospitals • Provide health system president/CEO with input and evaluation of hospital presidents• Adopt, approve and monitor management plans consistent with system strategic plan• Adopt, approve and monitor hospital operating and capital budgets consistent with the system budget• Approve and monitor asset transactions up to $2 million not otherwise addressed in hospital operating and capital budgets• Approve and enforce hospital medical staff bylaws and medical staff membership categories• Oversee and monitor physician alignment and medical staff development plans• Appoint and terminate hospital medical staff and grant/terminate hospital clinical privileges• Oversee regulatory compliance, accreditation and duties required by applicable laws and regulations to be performed by the board• In conjunction with the Community Benefit Committee, monitor and update the Community Needs Assessment as required by the Internal Revenue Service• Coordinate, inform and consult with Regional Councils, namely in the areas of:o Quality and patient safetyo Community benefito System financial conditionso System advocacy agendao Community engagemento Patient and physician satisfactiono Industry trends and best practices (in the form of continuing education)

We were able to improve our hypertension outcomes by over 15 percentage points, for example. • Patient satisfaction scores on CAHPS surveys are routinely in the 90th percentile. • Successfully transitioned the practice from being entirely fee for service to value based. • In April 2022 Ohio Family Practice merged with Pioneer Physicians Network.

I oversaw all the medical care being delivered to the hospice patients. This includes running the bi-weekly team meeting, called IDG. I medically managed patients and personally took care of them.

I medically managed all the residents of the facility, as well as employee health concerns.
