While assessing the risk of suicide in individual patients, in conjunction with treating clinicians. • Ensuring that providers follow-up on missed appointments for high-risk patients to ensure patient safety and in order to initiate problems solving about any tensions or difficulties in the patients ongoing care. The facility’s SPC and each patient’s principal mental health providers must work together to monitor high risk patients to ensure that both their suicidality and their mental health or medical condition are addressed. • Participated in the developed and implementations of ongoing education plan for providers, veterans, families, and member of the community on risk factors and warning sign for suicide and treatment options. Conducting group session with veterans who are part of a long-term treatment facility. • Primary Care Mental Health Integration (PCMHI) implementation within the Veterans Health Administration. Work with veterans and their Patient-Aligned Care Team (PACT). To help with their mental health needs.• Executed planning, implementation and evaluation of treatment program designed for medical, rehabilitation, and mental health patients as well as for patients seen in primary and ambulatory care clinical and in community settings including long term care facilities. • Preforming admission screenings in accordance with Veterans Health Administration. Consisting of MH RRTP Screening, MHICM, SW Assessments, and Demographic Screenings. Coordinated discharge planning, including providing information and referral services and accessing and coordinating community recourses/services; and providing case management.

• Utilizes the master treatment plan for assistance in identifying and implementing developmental age appropriate interventions with patients. This includes setting limits in a therapeutic and consistent manner on a patient’s behavior with the assistance of a registered nurse within program guidelines and identification of verbal and non-verbal cues for potential aggressive behavior and application of de-escalation techniques to the situation.• Implements appropriate interventions and documents patient’s responses in the medical record using the DAR format under the supervision of a registered nurse. Identifies thought patterns and behaviors associated with different types of psychiatric diagnoses and developmental ages and intervenes appropriately utilizing the master treatment plan

• Convenes and facilitates treatment team meetings. Demonstrates skills in assisting clients with I/DD service needs, selecting service providers and develops comprehensive individualized service plans based on the recipients needs. Supervised 20 Marines on a daily basis.
