Responsible for a 185-bed facility, mostly long-term care with a rehabilitation/skilled therapy section. Responsible for initial, quarterly, and annual social service assessments. Also responsible for quarterly MDS completions. I work with all physicians including dental, eye, and podiatry in helping to assess needs and schedule appointments. I work closely with families and residents to assist with problems or concerns including communication issues (especially during “Covid” time). I also work closely with the discharge planner/case manager for discharge planning. I complete PASRR’s. I assess residents for possible need of hospice and palliative care. I am an integral part of the interdisciplinary team. In addition, I work with many long term care programs in order to coordinate and assist residents after they are discharged from the facility. I provide weekly to monthly updates to case managers who are following my residents. I maintain a excellent working relationship with several home health agencies in the community

As part of the Behavioral Health Unit team, it is my responsibility to determine the eligibility of patients who are applying for short-term disability insurance. I interface daily with claimants and their providers. I am involved with team organizing and strategizing to facilitate patient assessment interviews. I contributed to redesigning the behavioral health forms currently in use. I assisted with training and development of new hires as well as sat with potential candidates and explained processes during the interviews. I successfully managed high caseloads of 90 and above and provided coverage during a co-workers time off.
