Leveraging the power of data sources through analysis to ensure actionable information is in the hands of experts responsible for quality outcomes and improved patient engagement/experiences, at the most optimal cost – in preparation for Value Based Pricing (VBP). The BHNNY PHM platform provides active care management and data analytics for predictive and risk analysis. BHNNY provides collaborative care management dedicated to a ensuring improved outcomes, particularly managing complex comorbidities and transitions of care.BHNNY also leverages technology assets and assessments to ensure incentive funding is appropriately applied; within BHNNY for oversight and predictive analysis, externally within the network of providers to incentivize change that operationalizes improved outcomes.
