Weekly PDSA, data generation of clinical indicators improvements, reviews among service providers, action taken and management. Clinical discussion: - Facilitate SN in the preparation of agenda and selection of case before the clinical discussion and draw the protocol based process map of case presented by the nurses and find out the gap in the process and action is decided on the basis of the gap. Infection prevention practices - Establish the different process of Sterilization like Autoclaving, HLD, Chemical sterilization, Chlorination and three bucket system of mopping. Ensure avability of sufficient number of instruments equipment and consumables after assessing the load of Clients. Postnatal counselling of Delivered women on maternal and neonatal danger sign by MAMTA in postnatal ward. Establish MDR and CDR in DH, Monthly review of maternal death and child death has been started to rule out the cause of Death. Effective management of Asphyxia- Build the skills of SN on the management of asphyxia within golden minute. Reliable 1st assessment - Preliminary test, History- Clinical and obstetrical, Physical examination is started. Implementation of Case sheet and referral and discharge card for better capturing the condition of PW. Quality services in FP: - Use of new consent form, use of protective attire, Sterilization of instruments, Follow up card and certification. Support GOI and GOB quality intervention like NQAS and KAYAKALP. Initiate WNB tracking and referral
