As the team lead, I oversee a team of chronic care managers, ensuring top-quality service delivery and efficient care coordination. My clinical expertise allows me to optimize patient care plans, monitor outcomes, and drive continuous improvements. I collaborate closely with healthcare providers to maintain high standards of care and comply with regulatory requirements. Committed to enhancing patient outcomes through innovative healthcare solutions, I focus on effective team leadership. My responsibilities include maintaining Electronic Health Records (EHRs), managing patient communication, scheduling appointments, handling prescription refills, and providing seamless administrative support. As a senior house officer in psychiatry, I assess patients through interviews, observations, tests, and medical history reviews to diagnose them accurately. I develop comprehensive treatment plans and prescribe medication as needed. My responsibilities include consulting with other medical professionals and psychologists, regularly following up with patients to check progress and adjust treatment plans as needed, and thoroughly documenting each patient's diagnosis, symptoms, medication, and treatment effectiveness. Additionally, I provide patients with emotional support and strategies to help them manage their disorders.

I implemented Procurement & Material Management's policies and processes to achieve primary healthcare center objectives. I was responsible for receiving and inspecting all incoming materials, comparing information on packing slips with purchase orders to verify the accuracy of shipments, and inspecting shipments for damages or defects. I recorded discrepancies or damages in shipments and reconciled them with purchase orders. I delivered received and inspected materials, supplies (medical and surgical), and equipment to the user’s section as requested. My duties included examining and inspecting stock items for any discrepancies in physical counts or defects/expiry, reporting any damage to the provider, and disposing of defective, expired, or damaged items accordingly. I ensured that all equipment/items were properly labelled and inspected. I issued items to various sections on a FIFO basis through an automatic replenishment system or on store issue vouchers and obtained authorized signatures for accepted items. Additionally, I updated the computerized store management system with details of materials received and issued, stored items/equipment in an orderly manner for easy accessibility, and prepared reports covering all store activities.

I conducted daily rounds of the dialysis unit, examined Electronic Medical Reports of patients, and ensured that the instructions of the Nephrologist were adhered to in all cases. I performed a supervisory role for Technicians, ensuring that all sterile precautions were met and patient safety protocols were followed regarding patient conditions, vitals, and the use of medical equipment. I attended to complications such as bleeding from fistulas and hypotension that patients developed during dialysis procedures. Additionally, I assisted Nephrologists in medical procedures such as inserting catheters and fistula/graft insertions. My responsibilities included conducting pre- and post-dialysis analyses of vitals, weight, and other relevant tests to identify patterns requiring medical attention. I ordered necessary monthly tests for dialysis patients and closely monitored and evaluated lab investigation reports to anticipate potential future complications. I also ensured that the dialysis procedure was closed as per protocol, with all information updated and monitored. In the absence of the Nephrologist, I received new patients in the Nephrology OPD, conducted initial diagnoses, and referred them to the appropriate specialist. Furthermore, I assisted the OT doctor in surgical procedures when required and supervised the implementation of emergency protocols in the absence of the in-duty Nephrologist.

I coordinated daily administrative duties, including managing schedules, assignments, and patient care. I was responsible for recording patients’ medical records and monitoring vital signs. Additionally, I supervised paramedical staff and oversaw their needs, ensuring smooth operations. My duties included overseeing patient admissions, transfers, and discharges, as well as mentoring and training new staff while providing support and guidance to all team members. I monitored medical charts, administered compassionate care, and assisted patients and their families. I liaised with specialists and administrators, communicated any protocol changes to staff, and ensured compliance with all health and safety regulations. Furthermore, I recorded and maintained accurate reports to ensure efficient and effective healthcare delivery.

I treated rheumatic conditions such as arthritis, back pain, and osteoporosis. My responsibilities included studying symptoms and evaluating their effects on the immune system. I designed tailored treatment plans and assessed joint-related conditions. I evaluated lab results and other advanced tests to provide accurate recommendations for treatments and advised on further examinations as needed.

I ensured all patients had complete histories and physical examinations during consultations. I supervised daily consultations of all OPD doctors and technicians, ensuring thorough history taking and physical examinations. I ensured that prescriptions and additional investigations were in accordance with established protocols and disease patterns. I also supervised the follow-ups of OPD discharged patients and malnourished patients on ambulatory treatment. Furthermore, I oversaw and ensured that all internal referrals adhered to the respective department's admission criteria.

I consulted with patients to understand their symptoms and health concerns, diagnosing and treating conditions such as asthma, bronchitis, and emphysema. I interpreted pulmonological exams and tests, including CT scans, chest fluoroscopies, ultrasounds, and bronchoscopies, to aid in the diagnosis of cardiopulmonary diseases and conditions. I performed various specialized procedures to acquire samples of the chest wall lining or the lungs for further analysis. Additionally, I provided immunizations against preventable cardiopulmonary diseases and maintained detailed notes of patient appointments, including comments, tests and treatments prescribed, and test results. I referred patients to cardiothoracic surgeons when necessary, advised these surgeons of patients' risk status, and recommended interventions to minimize risk, particularly in tuberculosis cases. I also provided support and advice to patients receiving long-term care.

I conducted daily ward rounds and served as the doctor on call at the hospital on a weekly basis, providing 24/7 coverage as per the scheduled call list. I carried out all emergency surgical and obstetric cases in the theatre and ensured proper documentation of patient diagnoses, treatment plans, and care on patient files. I attended and chaired morning handover meetings to provide feedback on patient care. Additionally, I handled all elective surgical cases twice a week in the theatre and filled out referral forms for both emergency and elective referrals according to standard operating procedures. I also managed patient data entry and documentation on EMR/EHR systems.

I gathered and verified patient information, insurance details, demographic data, and medical history via electronic forms or by phone. I used various software applications and electronic medical record systems such as Medix and Input Health, and had a solid understanding of medical terminologies and common medical procedures. Additionally, as the deputy team lead, I conducted a 2-week mentoring program for incoming team members, contributing to their professional development and the overall improvement and efficiency of the team.
