We can choose to do things differently. The time has come to raise the bar and aim for greater reductions in preventable disparities in health and development, as well as larger benefits for more people across racial, ethnic, and income groups, whether they live in urban, small-town, or rural areas. The first step should always be to eliminate disparities at their source by reducing poverty, increasing opportunity, and protecting people from hardships and threats related to racism and other sources of significant adversity. But we know that there is significant variation within groups in terms of responses to interventions and that it’s no longer enough to look at average outcomes. We need to notice and engage with those people who are not responding to standard approaches and think differently.In our trauma- informed coaching approach we use evidence from Center on the Developing Child, Harvard University and The Alberta Family Wellness Initiative to explain the biology of stress using straight forward psycho-education tools such as The Resilience Scale. Persistent, high levels of stress during the prenatal phase and the first three years of life- in the absence of an adult care giver able to provide responsive emotion validating relationships – can lead to physiological changes at a molecular level that can impact lifelong emotional, mental and physical health. As well as changes to the way brain architecture is laid down, systems such as the immune system, hormonal system and genetic expression can be affected. One of the reasons that people can struggle to make healthy life style choices as an adult is that their experience of a toxic stress response (with chronic inflammation) early in life can has caused changes to the functionality of their stress response at a molecular level and led to them being more emotionally reactive. Understanding this science and learning ways to cope with these differences can lead to improved emotional literacy which in turn can lead to improved executive function. With improved executive function people are able to make healthier lifestyle choices and make changes that they previously struggled with. When health, care and education settings apply the principles of person-centredness and work together with families and communities (as well as with other services), they can play a crucial role in improving lifelong health and wellbeing.Sarah has experience devising outcome frameworks that enable change and led The Somerset Emotion Coaching Project funded by Somerset County Council between2014 and 2022.

Project SpecialistEhcap Ltd Jul 2023 - PresentSomerset, England, United KingdomIn July 2023, I joined a small team in Somerset working with local practices and the Integrated Care Board to improve outcomes for people who had transitioned from one primary care setting into another. This involved creating an outcome framework for change and working together with colleagues across the primary health care team to improve outcomes for people with long term conditions. Measurable outputs were agreed at the outset.The trauma informed coaching approach enabled the team to connect with a common language and purpose and to develop self-care practices that enabled them to hold different conversations and more effectively engage with people with long term conditions who had historically struggled with change and making healthy lifestyle choices. This approach recognises from the start that we are all different and that some people need support building life skills in emotion literacy and executive function before being able to work effectively on prioritising and goal setting.This first project working with teh Somerset Integrated Care Board completed in February 2024 and I am now actively seeking similar work with Integrated Care Boards. -

Project Delivery ManagerEhcap Ltd Jan 2010 - Apr 2022DorsetIn January 2015 Bath Spa University and EHCAP were commissioned by Public Health , Somerset County Council to provide Mindful Emotion Coaching Training to schools and the children and young people’s workforce in Somerset. This project has now been extended through to March 2017. The training is based on the 5 steps of Emotion Coaching as described by Professor John Gottman and the neuroscience of emotion regulation - Professor Daniel Siegel . It draws on the attachment based evidenced parenting program Tuning in to Kids and the Attachment Aware Schools Project. All training includes working with the Mental Health Toolkit.

After completing my house jobs I took a role working with Community Services in Western Australia- based in an indigenous aboriginal community in Warburton. This was my first real exposure to a mindful way of being in day to day life and I consider myself very fortunate to have had the opportunity to learn from the elders I met at that time. On return to the UK I completed GP training. I was a partner in a practice in Fulham, London until I got married in 1996. I worked as a GP locum in Buckinghamshire until 2002 when we moved as a family to Cornwall. I opted to work in CAMHS until 2008 when I returned to work as a locum GP. -
