The underlying theme behind all of our stories is activism, and how it manifests in ways other than simply protest and direct action. We are long-time media pros (including former and current EICs, creative directors, and staffers from pubs including The New Yorker, Vanity Fair, BBC, The Washington Post, Esquire, New York Magazine, San Francisco magazine, The New York Times, T Magazine, Seattle Magazine, The San Francisco Chronicle, Sunset, Lonny, Wired, C Magazine, Curbed, 7x7, Dwell, and so many more) who have committed to taking pro bono assignments in the service of creating sustained, quality coverage of the issues and people that might otherwise "stop trending" over the next few months. •Reporting to the Chief Care Experience Officer, oversee patient and workforce improvement and intelligence to improve the overall patient/family/workforce experience of the very first patient and workforce centered SFGH department. This includes determining a best practice model, collaboration, enhancing first impression, creating standardized approaches to communication, professional development, workforce diversity, training and overall strategic guidance for the safety net hospital (50%).•San Francisco Department of Public Health Workforce Survey: Co-lead the efforts of first survey of SFDPH staff of 10K. This includes planning, dissemination, training, technical assistance, coaching, training, problem solving, resource allocation, strategic planning, and workforce development.•Write grants to secure sustainability of workforce and patient excellence: Successfully wrote and was awarded a 2014 Hearts Grant for $31,000+ providing funding to develop the SFHN Model and the 2014 Innovation Catalyst program, a partnership between Center for Care Innovations, Kaiser, and Blue Shield of California Foundation (10K) grant to address an issues as a result of the workforce experience survey.•FACES for the Future Coalition Lead: Oversee implementation for the SFHN’s first diversity pipeline project (Public Health Institute) that introduces low income minority high school students to the clinical and nonclinical professions of the health field.•Manage consultants and corresponding councils, workgroups and trainings. Supervise staff.•Member of the SFDPH Workforce Development Committee and Workgroups including: Large Scale Deployment and eLearning, Workforce Experience Survey, Black African American Health Initiatives Workforce Development, Professional Development, Academic Linkages and Internships.

•As an expert and founding member of the hospital’s first centralized learning center, provide consultation to various committees, leadership and departments for Health Reform through project design, implementation, evaluation, curriculum/training design marketing/branding and policy for strategic plan initiatives, systems, projects, learning and development. Through combined efforts within the department, 85% of the workforce, compared to 67 % in the previous year, has had opportunities to learn and grow.• Redesigned New Employee Orientation to meet regulatory requirements and improve the onboarding experience.•Restructured and oversee the Student Placements for non-clinical and clinical students to assure hospital compliance with the City and County of San Francisco. •Designed the curriculum/training, marketing/branding concepts, and managed the SFDPH system-wide implementation plan for Service Excellence (patient/workforce experience, HCAHPS/CG-CAHPS surveys, health reform) and re-launched the annual Staff Experience Survey (2012). 3,000+ SFDPH leadership, managers, staff, and providers trained.•Fair and Just Culture (FJC) Project Lead: Oversee training, curriculum design, and marketing approach for FJC, a SFDPH system-wide implementation for improved human behavior (report near misses and events) and system design (accountability and improvements). Efforts have resulted in a 12% increase of the frequency of events reported for the annual Agency for Healthcare Research and Quality (AHRQ) Survey on Patient Safety Culture within last year.•Change Management Project Lead: Responsible for researching and vetting potential change management consultants, evidence-based practice, and resources for leadership and managers.•Member of numerous hospital and SFDPH committees: FJC, Workforce Experience, Co-chair Schwartz Center Rounds, Service Excellence, Hospital Orientation, and the Quality Improvement and Leadership Academy Graduate (invited by CEO).

•Created a health education program through grant funds allowing for a needs assessment, program design, planning and implementation for a multidisciplinary center/geriatric clinic.•Managed health education clinic operations (1 Nurse Educator, 2 staff, and interns), patient flow, and service including individualized care and a drop in center to address, improve, or prevent chronic health conditions in tandem with providers, through diet, exercise, medicine adherence and motivational support.Created and administered a Clinic Patient Assessment of Health Education survey resulting in 92% of patients who were able to set goals to help them get healthy within the first year.•Administrative Lead for the PHASE (Preventing Heart Attacks and Strokes Everyday) Program: Kaiser designed/funded panel management program to reduce the risk of heart attacks/strokes for patients with diabetes, chronic kidney disease and various cardiovascular health problems. Oversaw the implementation of the first panel management program including medicine adherence regime, regular tests, checkups and structured patient education.•Managed quality improvement projects and worked with a team to move towards the Patient Centered Medical Home model (SFDPH wide initiative).•Introduced a Smoking Cessation and Asthma/COPD Clinic where seniors were provided one on one care with a public health nurse to quit smoking and/or improve breathing.•Held various positions including: Chair - Healthy Living Initiative Committee: Monitored and provided feedback for health education programming; Chair - Cultural Competency Committee: Designed a cultural competency evaluation plan to improve staff and senior relations. Voting Member- San Francisco Community Clinic Consortium Quality Improvement Committee. Member-Curry Senior Center Quality Improvement Committee.•Partnered with the SFDPH Health Education Section to develop and facilitate professional development training (learning and development).

• Provided strategic development, program design, planning, researched funding sources, wrote grants, and provided technical assistance for assigned clients.• Provided executive coaching and programmatic support to grassroots organizations and new non-profits.

• Managed clients including school districts, universities, city and county agencies, and national non-profits for a nationwide fund development firm. Trained and supervised three grant writers.• Provided personalized grant reports with potential funding sources for federal, state, and private grants with fundraising plans. • Wrote and edited Letters of Intent, applications and grant proposals ensuring grant compliance and client satisfaction. Successfully wrote awarded federal grants for clients.

• Designed, implemented, and managed the first statewide CIOT program, seat belt safety program funding law enforcement agencies in California administered by UCB. A $5.7 million grant.o 33% increase in seatbelt safety for California in program year one with law enforcement support, highest increase on record.• Created and maintained a monitoring system for mini-grantees, data collection and prepared various reports.• Recruited, hired, trained and supervised three staff and four interns to meet program goals.• Assisted with securing and maintaining awards for subsequent funding for the mini-grant seat belt enforcement program providing technical assistance for over 400 sub-grantees. • Developed, coordinated and conducted statewide CIOT trainings and statewide press releases.

• As a District Administrator, built a successful first year after school program for high school students and their parents from the ground up through a comprehensive plan created for programmatic success and rapid growth of student attendance and participation (800+ students).• Hired and trained staff of 15 to ensure effective program delivery. Collaborated and contracted vendors, agencies, non-profits and School Based Health Centers to expand service offerings.• Oversaw all operations, program budget, and grant reporting for federal grant. Designed and implemented marketing and outreach strategies for the program and coordinated special events to promote the program and celebrate accomplishments.

• Community Action Model (CAM)/Grants Coordinator for the SevenPrinciples Project Centers of Disease Control and Prevention Grant aimed to reduce infant mortality in the African American communities of San Francisco. Coordinated the granting process of 10+ mini grants, and provided technical support and CAM training (five step process to create change through building community capacity) for awarded non-profits, faith based and grassroots organizations.• Provided evaluation and grant management consultation for the Domestic Violence and Children Program, Intimate Partner Violence with Youth Grant to reduce youth partner violence. • Assisted with a social marketing campaign for infant mortality in the African American communities.
