• oversee wellness and health promotion delivery for over 50 clients nationwide• primary account liaison for clients, which includes but is not limited to scheduling and securing services to be delivered for existing clients, contract adherence and implementation; seamless coordination and success of the program or service; ensuring all contractual obligations are met; and attending client meetings, as needed• Responsible for providing leadership at client events, which includes but is not limited to assisting with delivery, answering questions, providing support and direction, and troubleshooting when asked or needed• Assist in the growth and development of new and existing health promotion programs and services• Supervise, train and create development plans for health promotion specialists• Coordinate the recruiting and supervision of interns• Coordinate the ordering of supplies for site services and service delivery• Compile, analyze and report program data, as required, by the Senior Account Manager• Preparation of invoice requests for deliverables• Submit financial projections monthly• Understand and safely perform the proper technique/protocol required as a screening technician/counselor for delivery of all Health Promotion products and services• Coordinate completion of event summary reports• Work to continuously improve the quality of health promotion programs and services

• Assist clients in accessing health related service• Coaches patients in effective management of their chronic health conditions and self-care• Facilitates communication and coordinates services between outreach and case management teams• Follows up with case management, outreach teams, patients, and providers to determine member compliance with instructed care, including home visits if needed• Leads the interdisciplinary team to coordinate care to assist with decreasing barriers to accessing care

•Recruitment of patients at the hospital to participate in the care transition intervention including obtaining consent for participation, rights, and consent to release information and providing copies of all consents signed•Completion of the Risk Assessment to highlight additional social or health needs that will used in assignment of a Health Coach•Facilitation of communication to Health Coaches regarding risks and strategies for patients referred•Facilitation of communication regarding dates of discharge and timely implementation of Health Care coaching•Discuss available resources applicable to the person post discharge including the local Aging and Disability Resource Center or other community based supports•Identifies resource and educational needs relevant to hospital stay and communicate to hospital staff•Collaborates with hospital discharge teams, community physicians and other providers to ensure smooth transition to the most suitable level of care•Promotes patient education and self-care management in order to reduce hospital readmission •Provides phone triage for any patients following discharge•Communicates with post-acute care providers to ensure best practice measures are followed•Tracks and trends readmission rates for the population assessed to identify trends that may warrant modification of the Risk tool used for the CCTP project•Works collaboratively with local Aging and Disability Resource Centers to refer for public services and programs available to the participant across the state•Coordinates clinical services and supports based on the resources available to the older adult to facilitate the individual plan of care implementation•Participants in care coordination meetings related to the West Baltimore Readmissions Collaborative•Assists in the development of policies and procedures directly related to the care transition coordination•Provides education to hospital discharge teams regarding community programs and resources

• Enroll and interview pregnant women to participate in a grant funded by the National Institute of Child Health• Interview participants and their partners to help determine stressors that can be associated with pre-term birth and other adverse birthing outcomes• Schedule and conduct interviews as part of the study’s data collection phase• Responsible for entering data from interviews• Provide feedback from the participants to ensure the study design fosters the key principles of Community Based Participatory Research• Responsible for planning and implementing the recruitment and retention events and activities for a large cohort study• Collect specimens from the mother participants

• Study hall monitors are responsible for conducting study halls by the guidelines set forth by the Tutor Coordinator and the sport’s Academic Counselor• Monitors must serve as appropriate academic and personal role models at all times• Monitors are also expected to set and enforce firm limits and act as an authority figure in the absence of the Academic Counselor• Academic mentors assist student athletes with developing positive study habits and organizational skills• Mentors also help students to set appropriate goals and to remain focused on academic success. Depending upon the needs of the mentee, most academic mentors work three or four hours per week, per student• Mentors may be requested for evening study hall hours, or during the day for individual meetings. Mentors may indicate the hours they have available, but once assigned, mentors are expected to meet with the student on a regular basis.

• Manage direct care employees who work in the residential setting• Meet with Case managers and parents to discuss goals for their children • Make employee schedule and provide group and individual supervision to all employees• Provide personal care to client by assisting with eating, bathing and dressing• Assist client with exercises to increase strength in hands and legs• Provide client with ways to redirect behavior when it is not appropriate for certain situations
