Telligen Community Initiative (TCI) is the charitable foundation of Telligen, Inc., a private, nonprofit healthcare intelligence company. Since its inception in 2005, TCI has provided financial grants to support organizations and projects that strive to improve the health of communities Telligen serves.TCI’s overall goal is to ensure opportunities for health are available and accessible to everyone. The foundation provides funding to health-related projects and organizations in Iowa, Illinois, Oklahoma and Colorado.

Accountable for organizational design, quality assurance, providing systems improvement, collaborating with diverse stakeholders, analyzing data, and program operations. Held the responsibility of developing operational strategies, planning, implementing, and evaluating program activities to improve community health initiatives.

Supervised the community team, responsible for managing education, health messaging in the community and navigation of clients across the continuum of care. Responsible for reviewing, improving or developing health promotion curricula and materials and training staff on how to deliver these messages to the community and through navigation.

Recruited, developed and facilitated communities of practice among participating providers. Performed root cause analysis, implemented QI strategies, and assisted with related systems improvement. Assessed transitions of care gaps within healthcare communities and recommended improvement strategies and processes to enhance beneficiary outcomes; assisted in the implementation of the methods and processes within the community. Analyzed primary and secondary data and information with community providers to identify gaps and assess effectiveness of intervention efforts.

Responsible for the assessment, planning, implementation and evaluation of disease management and wellness programs as directed by the health plan. Manage HEDIS groups and subgroups/ aiding in developing interventions for specific departments.

Designed and conducted community assessments to gauge community attitudes, awareness of health services; analyzed statistical health information, local demographics, social-cultural trends and other data to define the need for developing and distributing educational media. Planned and implemented educational programs, developing criteria, staffing and material requirements to reach program objectives, departmental goals and to address the public health problems of defined target groups; coordinated activities with other agencies and groups to achieve greater impact, avoid duplication and enhance the effectiveness of the public health education effort. Evaluated the effectiveness of health education within assigned areas of responsibility and ensured compliance with all grant responsibilities and requirements.
