About Daniel “Eli” Penn, Md, Fasge
An interventional gastroenterologist known for special dedication to his patients, a unique skill set, and innovative thinking. When a historic boon in endoscopic technology occurred during the decade after his fellowship Penn traveled and trained extensively to be among the first to acquire and hone new skills including endoscopic suturing, transmural stents, TIF, and third space endoscopy. His experience with endoscopic pyloromyotomy led him to a powerful realization about that patient population that has irreversibly changed his career. These patients with "functional" disorders had been seeing other specialists too, for other functional disorders. Once enlightened, all symptoms could be traced like ripples in a pool through immunologic, neurologic, psychiatric, and structural dimensions of every patient. Inappropriate mast cell activation, abnormal collagen, and autonomic dysfunction were the mechanisms. LHe cringed at the number of these patients who have been dismissed as psychosomatic after "normal" invasive procedures, with “IBS” prescriptions for fiber and dicyclomine, only to seek futile second and third opinions. The demoralization and guilt set in. We haven't come so far from "hysteria," afterall. Dr. Penn quickly became a leading provider for EDS patients in Georgia and an advocate for awareness. This was not well supported. He left and started a telemedicine platform for EDS patients around the globe. He is now starting a position in Beaufort, South Carolina where he will help build an EDS center at MUSC and will also continue to deliver extraordinary care to his patients. . He continues to enjoy being able to offer unique therapies to patients with few good alternatives (EUS, TORe, gPOEM, advanced resections, TIF).

Transoral fundoplication for GERD has evolved into an excellent treatment option for patients with small hiatal hernia with minimal hiatal disruption. Its current indication is for treatment of GERD symptoms or manifestations that do not adequately respond to acid suppression medications, but there is a growing debate about the implications of life-long PPI use and many patients are choosing structural repair to avoid having to take medicines, or to eliminate non-acid symptoms that would not respond to acid suppression in any case. Long-term outcomes are excellent in properly selected patients, and large herniae can be effectively treated with combination laparoscopic crural repair and TIF with excellent long-term durability and the benefit of a ~0% incidence of de novo dysphagia, that latter particularly important for patients with preexisting esophageal dysmotility. While some argue that a well-doen Toupet could serve the same purpose as HHR/TIF, the fundoplication created by the TIF has the benefits of being more anatomically correct and a lack of variability between one procedure and the next. Dr. Penn has performed neary 100 successful transoral incisionless fundoplication (TIF) cases, alone and in combination with surgical repair of the hiatus, and frequently serves as a speaker and proctor for Endogastric solutions.

Atrium-Navicent Health System, a large tertiary-care and level 1 trauma center offers nearly every service and specialty and is closely aligned with Mercer University School of Medicine and its residency programs. As a partner in a private group aligned with Atrium-Navicent Health System (in a PSA) Penn enjoyed collaborative relationships with surgical, oncology, and immunology colleagues and frequent teaching opportunities, but he perpetually struggled to get the attention of his partners and administration to address an inexplicable lack of work--in spite of high demand--as well as poor communication with patients and their providers, and inefficient utilization of office support. During the COVID pandemic his partners' productivity stayed level while he struggled to maintain 50% of his historic production. He was convinced that if he reached someone of significant authority they who would cringe at the waste, but this never occurred. An inexplicable discrepancy in access to procedure time persisted even after two years and his full partnership. He was still seeing patients that his partners refused automatically, and was then told what tests he could or couldn't order in their workup (such as hereditary pancreatitis panel, connective tissue panel), citing fear of confusing nurses and detracting from the focus on simpler GI disorders. In the fall of 2021 Dr. Penn began searching for other opportunities with more insightful partners and an administration more vested in its GI service line, patient communication, and where he is free to offer a full spectrum of GI care in the community he serves.

Chronic pancreatitis is exceedingly common and yet underrecognized due in part to the tendency for CT scans to show no obvious pancreatic abnormality in the 90% of patients who do not yet have calcific disease and the often normal levels of serum amylase and lipase during painful flares. Dr. Penn recognized the scope of our underdiagnosis of chronic pancreatitis while performing EUS on patients with unexplained epigastric pain. He is passionate about raising awareness about chronic pancreatitis, its association with tobacco use, and the incidence of hereditary pancreatitis in young patients who are often mislabelled with psychosomatic pain or drug-seeking behavior. He hopes to generate interest to benefit research on therapeutic treaments that can slow or stop disease progression.

SGHS is a 229-bed hospital that welcomed the addition of EUS and advanced endoscopic closure when Dr. Penn joined their small employed GI group in 2018. Oncologists and pathologists enjoyed the diagnostic and staging evaluations and Dr. Penn's attendance and participation in multidisciplinary tumor board. More advanced EUS-guided interventions, were not as well supported. Dr. Penn had been recruited by SGHS surgeons because of his expertise in managing postoperative leaks that had been transferred to him in Statesboro. He had even saved a young Hispanic man with near-complete intrathoracic anastomotic dehiscence following an Ivor-Lewis esophagectomy by an SGHS surgeon. Dr. Penn performed many memorable cases during his time at SGHS: acute Boerhaave closure, transcolonic drainage of a large diverticular abscess, T1 gastric cancer resection, and his first few POP/pyloromyotom cases. Ultimately the culture of the community-based hospital system proved untenable for Dr. Penn, however. He had no support in 2019 when an elderly woman with a large peritumoral abscess found only during EUS/FNA of a GIST tumor spontaneously ruptured into her peritoneum. Dr. Penn attempted to use LAMS to access and redirect the pus safely into the stomach, but was too late. DIC developed even as she was undergoing formal surgical resection of the mass and she did not survive. Although his actions were supported (and commended) by the board, politics effectively led to SGHS settling the family's demand for compensation. This betrayal led Dr. Penn to leave SGHS in the fall of 2019 to join his current group in Macon, Georgia, where he hoped to find a more supportive group of physicians and hospital.
