About Rex Mahnensmith, Md
I am continuing to serve as a Practicing Physician. From a 30 year span of serving in Medical-Center-Based Clinician-Educator roles, I transitioned to serving individuals and families in Skilled Nursing Facilities and then in a Federally Qualified Community-Baced Health Center, providing Primary Care, Urgent Care, Rehab Care, and Longitudinal Care of persons with various chronic illnesses. In these contexts, I had the capacity to take time with individuals - to sit and listen, to share concerns, and to deliver personalized care like never before. In this, I realized my true Calling - primary, holistic care of individuals in their unique social contexts - deeply assessing their burden of chronic illnesses, their acute health changes that they also were experiencing, their various weaknesses and fears, and their special, dependent needs. When I transitioned to the Community Health Clinic setting, I was able to connect and serve knowingly as each individual's Primary Care Physician. It is with humility that I have served in this chapter of my Vocation -- truly taking time to make deeply considered and personalized clinical decisions. I have aimed to serve carefully, expressing value, concerns, hopes, and direction to individuals as I deliver particulars of their clinical care. The privilege amazes and humbles me.I have hoped to confer a sense of genuine caring - caring for but also caring about the person, helping each within their specific social context. Serving in this way - carefully with caring - is my Calling. With this FHHC, I was privileged to serve as a Primary Care Internist to a large panel of patients. My service was Full Time. I engaged with positive anticipation each day of the week, providing Primary Care and Preventative Care to over 20 patients each day. I also assisted with Quality Assurance and Clinical Performance Improvement pathways -- which aimed to bolster and support specific health outcome achievements. It was an honor and a privilege - and blessing - to be serving with special colleagues in this clinic who shared these values and missions. I know that the patients and their families appreciated their care, as we connected and I got to know well their particular needs. I also served as Interim Chief Medical Officer during the early months of 2017. This was a very consuming add-on of responsibilities and roles - - - I departed from Stay Well in Spring 2017 on good terms to join my wife's family for her mother's Hospice Care in April and May in Indiana for six weeks, then returning to Connecticut.

Engaged with full time Internal Medicine Clinical Practice and Teaching from 1984 to 2014. Served as Practicing Clinician and Educator at Yale within the Yale Medical Group 1992-2014, at Brown University and Miriam Hospital 1987-1992, Medford Clinic Medford Oregon 1985-1987, Duke Medical Center 1984-1985. I love working with people, and I love patient care and teaching. At Yale, I grew a large outpatient medical practice that included primary care, hypertension, diabetes, various renal diseases, and associated co-morbidities. I also served actively as clinical educator to medical students and physician associate students in regular classroom modes and small group case study modes and mentored medicine residents and renal fellows regularly. Care of patients included in-patient and out-patient care, though most of my time was devoted to longitudinal care and urgent care in the outpatient setting. Over time, I have grown to love primary care of those with chronic illnesses and disabilities. Press- Gainey patient care reviews have been excellent. We moved north for family reasons to Simsbury CT in 2013 to live with and care for grandchildren. Thus I transitioned away from the YMG setting in New Haven to the greater Hartford, CT regions. I endeavor to use my skills and experiences to continue to serve individuals in primary care settings, whether in Ambulatory Care office, an Urgent Care setting, or within a Skilled Nursing Facility or in a Home setting. Serving others in the health care field as a physician is my calling. Beyond the special role of serving as a health care provider, I hope and aim to confer value and caring and encouragement to all the individuals who I have the privilege of serving. And, I similarly hope and aim to confer value and and encouragement to all the colleagues who I have the privilege of serving alongside. This mission is a special calling for us all - and mutual encouragement and affirmation are necessary ingredients.

In this capacity I had the special privilege of serving over a 15 state region - then a 30 state region as a Regional Medical Director for Gambro Healthcare. In this capacity, I was responsible for developing and directing Divisional quality improvement initiative and for teaching quality improvement methods to many community dialysis care groups across these states. I developed several pharmaceutical protocols and developed and taught problem-specific patient-centered clinical pathways. The intent was standardization of patient-centered clinical care processes and continuous quality improvement and care process improvement. With others, we implemented and taught standard means for assessing outcomes and analyzing gaps in outcome achievements - and encouraged corrective approaches to care that aimed to improve specific outcomes. This was immensely successful -- and recognized as "standard-setting" in the industry. I enjoyed very positive teamwork with Gambro colleagues.
