Provided case management to assigned clients to include: conducting assessments, service planning, linking, referring, and monitoring community services. Provide Mental Health Support Services that promote skill development, optimum functioning, and recovery. Provided crisis prevention and intervention services in a manner that supports dignity, recovery, and use of the least-restrictive intervention. Complete incident reporting, including human rights notification when required. Identify individual services needs through active outreach, engagement, screening and assessment activities. Assisted individuals with moving into other affordable housing in the community. Attend and actively participate in all Property Management/Support Services meetings. Review and respond to information in delinquency reports and respond to all lease violations by developing plans with tenants and property management to address issues.

Assist in the screening, interviewing and training of new Mental Health Technicians. Supervise staff in the completing of their daily routine and ensure daily and weekly paperwork is completed in a timely fashion. Assist with patients with such conditions as: psychosis, bi-polar, depression, schizophrenia, schizoaffective disorder, substance issues, Traumatic Brain Injury, ADD and ADHDServed as Mental Health Court Liaison representing Behavioral Medicine to various outside agencies and families as an Ambassador of Excellence. Manage groups that explore a wide range of topics such as functioning within community successfully and coping skills.

Assist with housing intakes including conducting interviews, gathering documentation, maintaining relationships with referral sources, and helping to insure that units are occupied by individuals who meet funding/oversight entity requirements. Maintain regular contact with tenants to ensure that individuals are engaged, needs are identified, and appropriate supports are provided. Provide direct supportive services, including crisis intervention. Assessing and testing to identify the client’s need for mental health and/or substance abuse treatment. Assisted the resident in identifying proper community resources to address areas of concerns. Assist with programmatic strategies to improve tenants’ economic self-sufficiency including financial literacy, budgeting, banking, and improving credit. Assist interested individuals with successfully moving on into other affordable housing in the community. Conduct street outreach to homeless individuals in the City of Norfolk in attempt to develop a relationship, complete face-to-face assessment and determination for P.A.T.H. Assess functional limitations, eligibility for service, and developed a Plan of Action with the client and, if appropriate, other individuals or agencies. Link and coordinate services and supports such as psychiatric treatment, housing, entitlements, medical care, skill training, etc.
